Service Agreement

All persons who choose to receive services must sign and submit a Service Agreement.  There are two ways of doing this: 1) You can print the Service Agreement directly from our Web site using your printer and then sign, date, and mail it back to Russow Consulting, or  2) If you do not have the capacity to print out documents, you may call (585) 442-4447 and request that we mail a Service Agreement to you.  When you receive it you will sign, date, and return it to Russow Consulting.

For those who do choose to print the Service Agreement themselves, our own printer friendly mailing form can be printed using your browser's (e.g. MS Internet Explorer, Netscape Navigator, etc.) print facility.  

After you print out the Service Agreement, sign, date, and return it.  A printed copy with your original signature must be mailed to Russow Consulting at 120 Allens Creek Road, Rochester, NY 14618.  You are welcome to expedite matters by faxing your form to Russow Consulting at (585) 442-7650, but the original still must be mailed:
